Required Forms

 If you are applying to be a first-year noncom, you need to submit the following:

Online references from three adults who know you well and are NOT related to you. The references you list on your application will automatically receive an email invitation to complete the reference form for you.

Once you are accepted, you will need to submit the following forms:

1. Noncoms and staff members who are over the age of 18 will be asked to complete a release for a background check.

2. Trading card information form. Complete this online through your Campwise account.

3. A high resolution picture of yourself that shows your face clearly. This picture will be used on your trading card. (Send this picture to Mark via email.)

4. Noncom Service Details form. Complete this online through your Campwise account.

5. Medical form. You may complete this online by logging into your Campwise account.

If you are returning for your second or third summer, you do not need to submit written references. We need the following forms from you:
